The complete SCA Sports Physical Packet must be completed and turned in to the SCA Athletic Department for a student to be eligible to participate in any athletic tryout, practice and/or game.  All forms must be completed by the parent.  The parent portions of the packet below can be downloaded and completed online. After completion, please print forms to add necessary signatures and either scan and email to jennifer.moss@fbns.org or print and send to school.
